One of the most common things I hear from clients in my Enfield aesthetics clinic is, “What treatment do you think I need?”
Sometimes a new client will come in with a treatment already in mind because they’ve seen it on Instagram or TikTok. They might be convinced they need dermal filler, a chemical peel, Morpheus8 or Sculptra because they’ve seen somebody else have an amazing result.
But before I recommend anything, I always assess your face, your skin and what is actually causing the concern.
Because the treatment that worked beautifully for somebody online might not be the treatment you need at all.
Finding Out What Is Actually Causing the Problem
When I assess your face, I’m not only looking at what we can see on the surface.
Ageing happens at different levels. We can experience changes to the skin itself, loss of facial fat and volume, reduced collagen and elasticity and changes to the underlying facial structure.
For example, pigmentation, dullness and uneven texture are mainly skin concerns. Hollowing through the cheeks or temples is a completely different issue. Loose or heavier tissue around the lower face requires another approach again.
This is why I don’t believe in treating everybody with the same procedure.
If the problem is coming from deeper within the face, repeatedly treating the surface isn’t going to give you the result you’re looking for. Equally, adding dermal filler isn’t going to correct pigmentation or poor skin texture.
The first step is working out what we’re actually treating.
When Your Skin Needs a Refresh
Sometimes we don’t need to add volume or dramatically change anything.
If your skin is looking dull, congested, dehydrated or uneven, I may recommend starting with treatments designed specifically to improve skin quality.
At Aesthetics by Marika, treatments such as our Oregano Peel and bespoke facials can be used to improve the appearance of texture, congestion and dullness while giving the skin a fresher, healthier appearance.
I often describe this as clearing the canvas first.
Healthy, smooth and hydrated skin reflects light beautifully, which alone can make the face appear brighter and more youthful.
When the Problem Is Facial Volume Loss
Then we have clients whose skin may actually be in good condition, but their face doesn’t look as full or supported as it once did.
You might start noticing flatter cheeks, hollowness, deeper folds or shadows that weren’t there a few years ago.
In these cases, treating the surface of the skin alone isn’t necessarily going to help because the concern may be coming from loss of deeper facial support.
Depending on the individual, this is where treatments such as dermal fillers or collagen-stimulating treatments may become part of the plan.
The aim isn’t to simply “fill” the face. It’s to restore balance and support in the areas that genuinely need it.
Does Your Face Need Tightening or Volume?
This is probably one of the biggest distinctions I make during a consultation.
Loose skin and volume loss are not the same thing.
And treating them as though they are can lead to an overfilled or heavy appearance.
If somebody already has heaviness through the lower face, continually adding filler isn’t necessarily going to create the lift they’re looking for. In some cases, improving skin laxity and tissue quality may be the better option.
That’s why I always assess the whole face rather than automatically reaching for filler.
When I Recommend Morpheus8
Morpheus8 combines microneedling with radiofrequency energy to target deeper layers of the skin and subdermal tissue.
I particularly like Morpheus8 for the face and neck when the main concerns include skin laxity, loss of firmness, texture or a heavier-looking lower face.
Rather than adding additional volume, we’re encouraging remodelling and improving the quality and firmness of the existing tissue.
For the right client, this can create a noticeably tighter, more refined appearance while still looking completely like you.
When Sculptra May Be the Better Choice
Sculptra works very differently from traditional hyaluronic acid dermal fillers.
Rather than simply providing an immediate filling effect, Sculptra contains poly-L-lactic acid (PLLA), which stimulates the body’s own collagen production over time.
This is one of the reasons I love Sculptra treatments for clients who want gradual, natural-looking rejuvenation rather than an obvious overnight change.
Results develop progressively as collagen production increases, helping to improve facial support, firmness and overall skin quality.
It requires a little patience, but that’s also part of its appeal. You don’t suddenly walk out looking like you’ve “had your face done”. The improvement develops gradually.
Quick Results vs Long-Term Skin Improvement
Another thing I always ask is: what are you actually trying to achieve, and when do you need the result?
Some aesthetic treatments work relatively quickly. Anti-wrinkle treatments, for example, usually begin taking effect within several days, with the full result developing afterwards.
Other treatments are deliberately more gradual.
Collagen-stimulating treatments such as Sculptra rely on your body’s own biological processes, so results develop over weeks and months rather than overnight.
Neither approach is automatically better. It completely depends on your goals.
Your Lifestyle Matters Too
There’s no point recommending an intensive treatment three days before your wedding, birthday or holiday!
Your diary matters.
Some treatments involve very little visible downtime, while others can cause temporary redness, swelling, bruising, peeling or flaking.
During your consultation, we can discuss upcoming events, work commitments and how much downtime you’re comfortable with before deciding on the most suitable treatment.
A good treatment plan has to work in real life — not just on paper.
Why One Treatment Doesn’t Always Do Everything
This is something I wish more people understood about aesthetics.
There isn’t one magical treatment that fixes every sign of ageing.
A treatment that’s amazing for pigmentation may do very little for volume loss. Dermal filler can restore structure but won’t necessarily improve poor skin texture. A tightening treatment isn’t designed to replace significant lost volume.
Often, the most beautiful results come from combining treatments strategically rather than trying to make one treatment do everything.
Combining Treatments for Natural-Looking Results
For one client, I might begin by improving the skin with an Oregano Peel or facial.
For another, we may need to address collagen loss with Sculptra or skin laxity with Morpheus8 before considering anything else.
Some clients may benefit from carefully placed dermal filler alongside treatments designed to improve the skin itself.
There is no universal formula.
The goal is always to treat the right layer, with the right treatment, at the right time.
That’s how we achieve results that look refreshed and balanced rather than overdone.
